CSHP, in partnership with Touro University College of Pharmacy, brings you the 2023 updated Virtual Advanced Practice Pharmacist: Chronic Disease State Management certificate program.

Activity Fee: $750 member | $1450 non-member

CE Credit Hour(s) (no partial credit given): 38 contact hours: 30 hours online home studies and 8 hours virtual and/or live activity. 

This program will focus on the management of common chronic disease states and issues including hypertension, diabetes, hyperlipidemia, anticoagulation, depression, anxiety, and safe medication use in older adults.

A combination of home study, clinical scenarios, patient interviewing skills, and hands-on practicum sessions will ensure each participant has the skills and competency to practice as an advanced practice pharmacist. Upon successful completion of this program, participants will be capable of: 

  • Performing patient assessments
  • Ordering and interpreting drug therapy-related tests
  • Referring patients to other healthcare providers
  • Participating in the evaluation and management of disease and health conditions in collaboration with other healthcare providers
  • Initiating, adjusting, and discontinuing drug therapy

Your successful completion of all online home studies and the live event performance tests will meet the certification portion of the criteria for becoming an Advanced Practice Pharmacist.
